Who We Are
Keystone Community Care is a team of dedicated disability support workers based in Brisbane and the Gold Coast. We specialise in providing personalised NDIS core supports for self-managed participants, helping people live independently, connect with their community, and achieve their goals on their own terms.
What makes us different is our genuine passion for the work we do. We are not a large corporate provider — we are a close-knit team that takes the time to truly understand each participant's needs, interests, and aspirations. We believe that the right support worker match can transform someone's quality of life.
Our Values
Person-Centred
Every support plan is built around the individual — their goals, preferences, and lifestyle choices.
Community Focused
We reinvest in local disability programs and community initiatives across Brisbane and Gold Coast.
Qualified & Screened
All team members hold NDIS Worker Screening, First Aid, CPR, and relevant disability qualifications.
Passionate & Energetic
Our passionate, dedicated team brings energy, creativity, and genuine care to every interaction.
Meet the Team
Keystone Community Care was built by two people who saw an opportunity to do disability support differently — with heart, consistency, and a genuine commitment to every participant's wellbeing.

Heidi Griffiths
With more than 13 years of hands-on experience in community care and operations management, Heidi founded Keystone Community Care to bridge the gap between large corporate providers and the deeply personal support that NDIS participants deserve. Her career has spanned frontline disability support, team leadership, and service design — giving her a rare perspective that combines operational expertise with genuine empathy. Heidi leads every aspect of Keystone's operations, from participant onboarding and support worker matching to quality assurance and community partnerships. Her philosophy is simple: treat every participant the way you would want your own family to be treated. Under her leadership, Keystone has grown into a trusted provider known for consistency, warmth, and going the extra mile.

Samanta Pinto
Sam brings over five years of dedicated participant support experience and a natural ability to connect with people from all walks of life. As Co-Founder and Senior Support Manager, she is the heart of Keystone's day-to-day service delivery — working directly with participants to build independence, confidence, and community connection. Sam is known for her calm, attentive approach and her talent for turning everyday activities into meaningful progress toward each participant's goals. Whether it is supporting someone through their morning routine, navigating public transport together, or helping a participant try a new community activity, Sam brings professionalism, patience, and genuine care to every interaction. Her commitment to person-centred support sets the standard for the entire Keystone team.
